I use Squid for user accounting with Radius for my LAN,

after searching on google i found this :

authenticate_ttl 1 hour
authenticate_ip_ttl 10 seconds
acl ip_unico max_user_ip -s 1
http_access deny ip_unico
server_persistent_connections off

and added to squid–> General --> Custom Option

its works great.
