Unable to SMTP in??

Recently implemented pfsense at a client. It's working great other than for SMTP.
I can "telnet [hostname.domain] 25" & ping the server from the inside.
I can "telnet [local IP] 25" from the pfsense box
I cannot "telnet [hostname.domain] 25" from the pfsense box. The pfsense box translates to the public IP but still won't connect.
From the outside I timeout with the telnet to the hostname.domain or public IP.
attached are the WAN rule, NAT port fwd rule, & I've disabled smtp QoS for the time being.
The server gateway is set to the pfsense box. It has local & public DNS assigned.
This server has multiple other services (web,ftp, ICMP) which are performing correctly.
hosts file has the public IP listed as well.
1 part fixed:
mail server is qmail with outgoing queue " delivery 3: deferral: Sorry,I_wasn't_able_to_establish_an_SMTP_connection.(#4.4.1)"

Where are you trying from exactly? Many a isp block 25 outbound from their network, and limit traffic to their mail servers.. So just because you can not get to it doesn't mean its not open..
Did you go over the troubleshooting doc? Pretty much first step is actually verify traffic is getting to pfsense wan.

I've contacted my ISP they're not blocking anything.
well this is odd, I'm seeing tcpdump connections from external server but I cannot "telnet [host.domain] 25" as that times out.

What you're describing indicates it's a firewall on the destination host itself. Or, perhaps, an MTA config that only accepts connections from local networks.
