      hello.
This is my first post and my English is a bit rusty.

      I will describe my scenario:

I have a virtualized private network with Hyper-v inside my computer with w10 (192.168.111.2). This network communicates with the internet with a virtual PFSENSE which has a fixed IP as a WAN network (192.168.111.103) within my real network and 111.111.111.1 as a LAN network

The private network has the sql server (111.111.111.101) and a w11 client (111.111.111.102) connected to it which works perfectly.

I need to access the SQL server with my physical computer (192.168.111.2)

I have created a NAT with port 1433 and destination 111.111.111.101 but it didn't work.

      What am I doing wrong?

In the WAN interface settings you have to disable "Block private networks". Did you do this?

BTW: you should not use public IP address spaces in local private networks.
