Cannot generate Vouchers

Hello,
I am using latest pfsense version, i activated the voucher option in the captive portal but i see no generate vouchers button. I have tried in 3 different browsers (IE, Chrome and Mozilla).
Is there another way i can generate the vouchers? Is there a file where you can add or manually generate vouchers?

The doc talks about a lot of things :D
But nothing about this button:

After clicking the indicated button, you will see a small blue round button next to it. Click it to see your vouchers-code.

This is what i am trying to explain, i don't see that freaking button. I have version 2.2.2 stable.

Although i cannot see it in the webpage, i was able to find the button in the code and work around it.

No "+" button ?

As soon as i check "Enable Vouchers" and Save at the bottom of the page, the "+" appears.

What version of pfSense are you using ?? Any packages installed ?

If it is 2.2.2 without packages, consider you setup as broken. Reinstall…..

As i said in the first post, i am using the latest pfsense version (2.2.2 Stable). I didn't have any issues while installing so i don't see it as broken. The only package i installed was nano (i really hate vi).
Yes, i don't see the plus button and while i was able to workaround that missing button and use the code link, vouchers still don't work. Even when i try testing box freezes.
Also, i am using pfsense in virtual enviroment (VirtualBox).

A browser cache problem ?
The image file $g['theme']}/images/icons/icon_plus.gif => /themes/pfsense_ng/images/icons/icon_plus.gif exists ? It has the correct owner and rights ?
