Fix Bug #4326: Limiters & Squid Proxy Conflict

This bounty would be to fix the issue described here:

I'm able to offer $150, but I'm hoping others could join in with this as I'm sure bandwidth limiting is important for many others.
The successful project would:
Allow a pfSense admin to limit inbound and outbound traffic using the existing limiter functionality while squid transparent proxy is used. Currently, only inbound traffic is limited and outbound traffic grinds to a halt when the limit is hit.I'm not foolish enough to think my $150 would get this done, so that's where I'm hoping others would join me. Also, According the the redmine ticket, it's on the road map anyway, so this is a double win.

i guess the squid package could automagically generate a rule for each selected interface.
unfortunately, not everybody would agree with this.
It might also be tricky to be sure the rule is always on TOP.
