SquidGuard + LDAP Authentication issue

Greetings friends!

I have issue on Squidguard wherein it will stopped the service when using the LDAP option. My suspect is, it cannot authenticate or is it a connection issue?? It can somehow authenticate in SQUID proxy user authentication and diagnostics LDAP authentication but cannot in Squidguard LDAP option.

In Squidguard LDAP option it only required LDAP DN and password while other LDAP authentication needs IP address or Base DN. Thus I tried putting the IP address and the port but still no avail. I don't know if it's the correct syntax??

Any feedback/help on this issue is greatly appreciated.
