      Good day,

I have pfsense setup virtualized on a win 10 computer with virtualbox, setup using the netgate guides.

      Version is 2.5.2

I can only access the web GUI or the internet if I have the LAN side connected to a switch.
I'm also getting terrible internet speeds; about 30 Mbps on a 1,200 Mbps connection.

I would expect that the host computer should be able to access the GUI without the external switch at the least.

Am I missing a configuration item?

It completely depends how you have the VM configured in VBox. If the host NIC is passed to the VM then it would not have a connection using that and would need some other connection via the VM.
