TCP_REFRESH_UNMODIFIED ACCELERATOR RUNNING?

Hello fellow netgate community members,
I recently have Squid working with HTTPS and it is caching showing misses and TCP_REFRESH_UNMODIFIED
This is the definition of each of what the cached items can be,
TCP_MISS - Object not found in cache, downloaded
TCP_HIT - Object found in cache.
TCP_NC_MISS - The request was made for an object that can't be cached.
TCP_REFRESH_MISS - Appliance had the object in cache, but a check with the CACHE (GET with the If-modified-since) indicated that the object was stale. Object was downloaded from the CACHE and the new object was placed in cache.There is Miss however none show hits, it seems like it has been disabled somehow. Can you please help?
I do get TCP_REFRESH_UNMODIFIED sometimes
I was reading that SQUID must have chmod done on a file, is this the problem? Let's get this accelerator running again, it's amazing to watch it run when you add the certificates and fine tune it I feel I am so close to getting it perfect.

Also is the cache encrypted and using the onboard Hardware crypto
