OpenVPN TOTP - 50X error on RADIUS authentication

I’m working toward adding TOTP to my OpenVPN connection on an SG-1100 but cannot get the RADIUS authentication to work. I set up the RADIUS server, added a user, and then tested authentication under Diagnostics ... where I get a 50X error, stating:

The web server encountered an error processing this request.

The crash reporter gives:

No PHP errors found.

No FreeBSD crash data found.

All packages are up to date and this happens both on 24.11 as well as the 25.03 beta.

Given that others haven't posted something similar I must assume it is an incorrect setting somewhere on my part, but I am at a loss as to what that might be. Any ideas?
