Some things Port Forward , Others dont :S Confused

A bit of background
Been playing with PF sense for a while , mainly for web filtering ads (via PFblocker)
I have had no problems with this set up for many months . I have a simple flat network , one IP range
I have Coax coming into my house , connected to a Cable Modem (not routing - modem only ) connected to Wan port of PF sense
Lan 1 Port plugged into a Cisco Switch , with all management features switched off (effectively a dumb switch)
I have a bunch of devices plugged into the cisco switch , such as Smart TVs , WiFi access point , NAS, Computers, Laptops
I can foward ports to My NAS just fine , even UPNP works with this , for example if I want to grab a document I can connect remotely to my Nas using Port 8080 or 8081 no problem
Short story short is my Desktop ( connected via Ethernet) has problems with Port forwarding games .
Long story is recently I started playing a game called Grim Dawn , due to covid outbreaks. Strangely I can host and clients can join (not sure how since the manual specifies UPNP to be enabled and I disabled it for security reasons) though the game has some major De-sync issues . So I tried to manually set up P2P game servers and following the manual but there seems to be a problem when trying to host a game server , the port will not appear to open. It seems either a listening problem or port forward , not sure .
With Grim Dawn running, I opened all ports found from the Netstat command, including the game launcher also including those found in manual
When I run a port open test (yougetsignal.com and others ) the NAS ports still show open but none of the Game ports show open .
I have tried this with a few other games and getting the same issueI have tried lowering the windows firewall , this makes no difference . I also tried temporarily enabling UPNP but same issue remains . The game has UPNP also but fails to Forward to PF sense ( yet my NAS does )
FWIW I could get ports to show open for Windows machines when using a utility called 'Port listener' found here, just the port wont open when a game is running . :s
Any advice would be appreciated as I am stuck at home haha
