Inbound (server) load balancing (WIP)

The load balancer (inbound only now due to changes in the outbound balancing) is being revamped and is a work in progress.  I will post a reply to this thread once it's ready for testing (and more importantly…bug reports - of which I'm sure there will be some).  Thanks
