FreePBX behind PfSense...working, but only halfway there.

Had a need to replace my failing firewall (USG Pro4), went with a Netgate 4100. Flat network. Internet is via a 4G cellular (going strong for nearly 3 years) Teltonika RUT240 with a static IP (T-Mobile) in IP Passthrough mode. Network has been incredibly stable, with some of the best up/dn and latency numbers I've ever had. FreePBX has been nothing short of rock solid for nearly 3 years (outside of issues that have been self created/corrected).
Rolled the new PfSense 4100 in place everything 'appeared' to be fine.
Here's the problem (and reason for the halfway there title). Inbound calls connect and process just fine...including audio...just the same rock solid performance. However, outbound calls connect, but no audio will pass (outbound ONLY). In my testing, as long as the receiving party answers, the call connects, and stays connected, just no audio (again, outbound only). If the receiving party calls back, everything is perfect (audio passes fine).
I've worked a couple weeks deconstructing/constructing the NAT rules, pouring over Googlefu searches, applying different settings with no alteration in the outbound audio passing through. I know it's capable, inbound works without issue.
Look forward to any ideas that may help with this last bit of configuration.
