Boot environment, packages, and settings revert after reboot

I'm posting here in hopes of troubleshooting my SG-1100. I am having trouble getting my settings to remain after a reboot. I used the appliance without problem for 3-4 years, so I worry that it is an issue with the storage, but hopefully it is a setting I overlooked.
System: Netgate 1100
Version: 23.05.1-RELEASE (zfs install)Currently, if I create and activate a new boot environment like this
Then reboot to activate the new environment, and find that the newly created boot environment is gone, like this
Finally, even without creating a new boot environment, if I update, install, or remove any settings (including interface settings, firewall rules, adding and removing packages) I find them reverted after a reboot, making it difficult to make my network function as intended.
Does anyone know what may be causing this issue, or any troubleshooting tips that could help?

From the command line I ran
zpool status -x
which resulted in the following outputpool: pfSense state: ONLINE status: One or more devices has experienced an error resulting in data corruption. Applications may be affected. action: Restore the file in question if possible. which indicates that the pool must be destroyed and restored from backups. Seeing as how the SG1100 disk is not user replaceable, I will try a software reinstallation and hope for the best.

Yes backup the config and try re-installing clean to be sure.
