I have 1 WAN and 1 LAN interface, During the setup, I allowed all the traffic from LAN to WAN..
In pfsense host itself, can connect to all internet destinations and services without any problem.
But from my client in LAN, only can resolve DNS, but all other traffic was denied by the firewall.
With packet capture, it showed me the follows:

      18:12:57.980209 IP 192.168.1.219.2239 > 210.17.245.148.80: tcp 0
      18:13:00.843054 IP 192.168.1.219.2239 > 210.17.245.148.80: tcp 0
      18:13:06.864814 IP 192.168.1.219.2239 > 210.17.245.148.80: tcp 0
      …. repeating for all traffic...

anyone got clues?  Which rules did I miss?

      Thanks.

We need more details. The best thing would be, if you post the content of /tmp/rules.debug here or attach screenshots from your rules screen…

It suddenly work somehow. By the way, I realized there are 2 ways to assign gateway, one is in the "Interface" Menu, second is in the "Routing" Menu -> Adding Gateway -> Adding Route.  What are their difference?

I found that one the first way work. It is strange.

          Thanks.

There isn't one. Both add gateway entries.

I just finished troubleshooting a similar issue. It seems that once pfSense was installed, there was no gateway setup. We had to add the gateway, and then select it as "default", then reset the firewall state. At that point everything worked as expected.

The odd thing was that we had a 1:1 inbound mapping setup, and that worked fine without a gateway. This definitely threw off our troubleshooting path a bit.

              Hopefully this helps someone.
