I have purchased pfsense Gold and have the book/documentation explaining the various Virtual IP options but I am still confused. I have 5 IPs give by my interent service provider, they are all static obviously. The WAN is using one and I want to assign another to a machine connected to pfsense so that I can do NAT port forwarding. Would I select Proxy ARP or Other in this case?

I do this with IP Aliases and it seems to work well enough for me.  I have 14 addresses with 1 for WAN and a few others for WWW, FTP, Email, etc.  I have pfSense assume those IP addresses and forward them through to the real servers on LAN.

So according to this it appears I should use IP Alias?

You should be using a VIP type that has the characteristics that meet your requirements.

DO NOT set a source address/port in a port forward or firewall rule unless you KNOW you need it!

Well I only need NAT to be working and IP alias does that, Proxy ARP did not work so I will use IP Alias for now I guess

IP alias is fine. Some modems aren't happy with just proxy ARP, which sounds like the case for you.
