Pfsense & Hosted VoIP

Dear Guys, I hope you may be able to advise on this very strange issue we are experiencing. :o
We are running 3x pfsense at our 3x offices, we all have 1 phone at each office and we are using the Gamma Horizon VoIP platform
With our old VoIP provider we never had any issues but since changing providers we are experiencing wrong names when calling our internal extensions and our provider can not sort it!
We all run Cisco's SPA504g and when we delete all caller history the phones may then work correctly for an hour but then this is what happens.
Peter dials John on ext 103 the phone displays that he is calling Richard
Richard calls John on 100 and his phone displays that he is calling PeterEven though the phones are displaying you are calling the wrong person it will actually connect you to the right extension
Nothing really follows suite as sometimes the name changes randomly who you are calling.
I am just wondering if anyone has ever experienced anything like this before or if there is any configuration settings we may be able to try.

The names displayed on the phones has no relation to the network or firewall. If your calls succeed, that's the extent of the influence anything network or firewall related can have. Something on the phones or PBX is causing that issue.

If you can find out what voip system they are using that might help you track down the problem.
EG in freeswitch an opensource voip system which can do landlines as well, like Asterisk, you tend to have the name associated with the extension DID, eg:

Until you can find out just how they are associating names to an extension, there could be any number of possibilities in play.
For example, do you have your workstations plugged into the phones to minimise cabling and do you run vpn's for file sharing between offices?
Have you switched on logging in various rules in pfsense to following the traffic in the fw logs?
If you get nowhere with then, trying swapping out pfsense for a basic ISP supplied router (if you have any) and see how things work then for a short period of time once the problems show up. If it persists its easier to point the finger back to the VOIP host.</variable></variable>
