Almost any Huawei 3G/4G modem (USB dongle) should work.
The modem should be properly configured before use.
Get the modem and I'll advise further. -

Huawei E398 should work. You may find a better (cheaper) option buying locked or operator branded modem from the local mobile operator store. Unlocking is not a big deal.
I'm pretty sure we can avoid using usb_modeswitch.

These sticks are intended for home/office usage, and they behave like a USB CD-ROM containing the drivers to be installed in Windows, when plugged in first. After the user installs the drivers properly, each time the stick is connected, when it mounts as a CD-ROM, the driver recognizes this and switches it to a serial or a network interface and connection becomes possible.
In linux and bsd, the situation is the same. usb_modeswitch is a tool which does this for the supported modems.
Some Huawei and ZTE modems have the ability to disable their CD-ROM mode completely (using special AT commands from a terminal) and appear as a modem straight from the beginning when plugged in. I've had a recent experience with an Android device which had usb_modeswitch implemented in a very strange manner, the 3G stick refused to work until I re-enabled CD-ROM mode in it, seems that usb_modeswitch is actually required to exit successfully in order to dial up a connection.
It all depends on the USB stick model. -

What about Wifi hotspots tethered to a pfSense box via USB - does anyone know if that will work as a 4G modem? My device is a Sierra Wireless 763S.
Or am I better off putting a wireless NIC in my pfSense box and then joining the Wifi netword of my 4G wifi hotspot for my WAN connection?
This isn't something I would use on a regular basis - just when my regular cable internet is down. It was down for about six days two years ago do to an ice storm and it was also down for a couple of days due to a cable cut from water line construction.

I never owned, used or recommended it. Personally I would recommend Huawei E3372H as a replacement. Fortunately some people already posted here some receipts on using it in the different modes.
