Are you trying to install to a USB device attached to a USB hub?

I plugged the USB drive directly into the server and when I used a CD I used teh DVD drive that was installed in the server.

Go through BIOS settings with fine tooth comb. I typically turn off everything that is not needed. A couple of items to pay particular attention to are USB "legacy/emulation mode" and USB boot enable.
Legacy/emulation mode may be called something else in various BOIS's. But it is essentially whether or not the BIOS continues emulation after a USB capable OS is loaded.

Just out of curiosity, do allot of people run pfsense on servers? The minimum system requirements are so low what is the purpose of putting it on a power edge?
I'm guessing its for some server nic cards.

You must be using a logitech usb keyboard or usb mouse?
Something "logitech" isn't playing nice and you need to find something else to use besides it.

Many of us rely on pfSense in production settings, organizing entire racks of physical servers, and even hundreds or more virtual instances within. ;)

Thought the system requirements are low in most cases I am in a server envirment and need as much throughput as I can get.
It is time to update our routers and instead of playing $3,000+ for one router I would rather use a retired system that meet our network requirements.

To save money we have not installed any server grade KVM's in our racks but instead have a small home user KVM that supports only 2 computers. This way we can work on more that one system at a time if needed.

Just a FYI, I have pfSense running on a few Dell 860's, 1425's and 1950's all without issue so it's definitely doable.

Turned out it was my Logitech M100 mouse. Unplugged it and the problem vanished.
Now my only problem is connecting to any servers through the WAN network.
