I bought a Firewall micro appliance from amazon with pfsense pre installed. How do I know the pfsense install is genuine and does not contain modifications of malware?

If it's purchased from the official pfSense store or Netgate (company behind pfSense project) then you don't have to worry. Everything else is not official and can expose you to the risk. Mind telling us from which seller you have purchased the appliance? Preferably link the item.
Like Doktornotor said, only way to be sure is to reinstall pfSense.

From what I read of that link, it says nothing of any os or distro being pre-installed. Just lists firewall distro's its compatible with.

Yes, It came with and older version and I updated it through the web interface.

One of the reviews stated that it comes with pfSense pre-installed. At least the vendor is smart not to advertise it as a "pfSense firewall" as you so often see with similar devices.

I reinstalled pfsense and the web interface is back to defaults. I downloaded the latest stable 2.3.3-RELEASE but looking at the dashboard version 2.3.3_1 is available. Is the website not up to date?

current is 2.3.3p1 - not the "release" is 2.3.3 so yes you need to do an update after a clean install to get you to p1
