The pfSense Book - can we get a permission to print it in book format for personal use?

I really like The pfSense Book and would like to have it in book format.
I want to submit it to Lulu to have one copy printed and bound as a Private Project but there is a copyright notice right on the first page of the PDF. They will likely refuse to print it even though the PDF allows printing.
Can we please get a one line notice underneath the copyright that printing for personal use is permitted.
Better yet, can you submit it yourself as a self-published project so that we can all purchase the book on paper.At Lulu the approximate prices for the ~650 page book are:
$22 - Standard Paperback 8.5x11in
$23 - Premium Paperback 8.5x11in (glossy cover)
$30 - Casewrap Hardcover 8.25x10.75in (nobody likes dust covers and they are more expensive so the Casewrap is better; see example here)Since the book is already written, all it requires is a nice cover (to judge it by :)

A single copy for yourself might fall under fair use. What I'd prefer though, is an epub version, so that I could have it on my tablet.

@JKnott ?? It's there... But compared to a hardcopy, I prefer the living document.
