IGMP - RECV unk: 0x22/0x00 from 192.168.16.101 to 224.0.0.22

Hello everyone. I've been trying for weeks now to get IPTV to work correctly on my network and I'm about to pull my hair out. I've read dozens of threads on this issue but have not been able to find an answer.
Here's what I've done so far:
I've got the IPTV running on VLAN16. It grabs an IP from the DHCP server just fine and actually will display a few seconds of video before stopping, so I assume my network is setup correctly and it's an IGMP proxy issue.
In my syslog, I get this new entry with each channel change (192.168.16.0 is my vlan16 subnet):igmpproxy[50558]: RECV unk: 0x22/0x00 from 192.168.16.101 to 224.0.0.22
I have updated the igmpproxy pkg as mentioned in other threads but no success.
I've attached my upstream/downstream config in a screenshot.
Firewall log is empty.
I've also attached the IGMP settings in my switch. All other igmp related settings are default.Any ideas?

I believe I have narrowed it down to how igmpproxy sees interfaces.
First I made sure everything was working fine without using any vlans. Got that working as it should.
Then I configured my switch and pfsense in the same method except using vlans this time. Here is where I think it is having trouble -
I've got my virtual interface tagged with VLAN15. When I assign the desired interface to the plain interface without vlan tagging from esxi (i.e 'em2' in this case), I see the unk in the error logs. I don't think the 192.168.15.1 communicates with igmpproxy correctly when looking at the logs. My IPTV does grab an IP and shows a few seconds of video.When I switch the interface to 'VLAN15 on em2', I see the correct membership queries in the logs. (e.g RECV Membership query from 192.168.15.1 to 224.0.0.1). However, I'm unable to get my IPTV box to register an IP on that subnet. How can I get my iptv box to get an IP? I've tried assigning a static IP but that does not help.

Your device might not support vlan tagging so can you get your switch to tag the packets/frame as they come in and strip them as they go to the device?
