VOIP- strange problem with incoming

OK, a rewrite of this post with my new findings. I have configured pfsense to how I would assume it needs to be configured to allow inbound SIP calls.
I use a epygi quadro for my IPpbx.
SIP is configured on port 5060 UDP and RTP from 6000 to 6090
from what I can gather my provider uses a proxy on their end to proxy the SIP traffic.
I have stun configured and it appears to be stunning for the most part - I dont really know how to check.Secondly I am successfully able to make incoming calls if I move the IPpbx in front of pfsense, but behind my cisco router. My cisco router is forwarding ports correctly.
I have had this pbx working perfectly with the same configuration it has now behind IPCOP instead of pfsense.Now for the problem. It is very difficult to identify what exactly is happening, but when someone calls my SIP number they should be greeted by the auto attendant, as I said before this works perfectly when pfsense isn't in the picture.
However when I call my SIP number and the IPpbx is behind pfsense I am greeted by a male voice (still part of my pbx) and told that my call cannot be connected at this time.Please can you look over my configuration and see if you are able to pick out anything. I have looked all over this forum and goooogle and found nothing. I have found ppl with a similar problem and the resolution to the thread was, "thanks its working now, I don't know what I did".
btw my IPPBX (quadro) is @ 192.168.6.3
