Freeradius + OTP sometimes auth failed when auth openvpn.

Hello,
I have test freeradius + OTP to auth openvpn connect.It's can connect and auth it.But it's has sometimes auth failed.I have go to Services - FreeRADIUS -Settings don't change any settings and click Save.It's can use freeradius + OTP auth.What's this problem?

I have seen the same behavior in my pfsense.
I have two implementations with different versions and both feel the same.
You find some solution?

I only re-click settings on radius and click save it.It's can temp use it.But I don't know what time will failed.
