How can I remove a previously created  "Custom Access Restrictions" from the NTP service?

Looks like the delete button has been forgotten? :o

    (edit: pfSense 2.3.3-RELEASE-p1 (amd64) )

Clicking Add to see a Delete button is not a valid fix.

Plus, the "when you click the icon on the button then the damned button does nothing" fix has apparently never been backported to 2.3.x:

Unfortunately there is no simple solution to this (a solution that would not require touching every UI page that uses the "row helper").

In general (see Firewall->Alias) there must be at least one row in the table because adding rows duplicates row 0. If you don't want any custom restrictions, just clear the "Networks" Element before saving.

I will make a change to ensure no errors are reported in this case.

In general there're way too many pointless broken JS gimmics across the GUI. And yes, duplicating the gimmics code over and over and over again is another issue.
