We have a 40/10 Mbps VDSL2 connection. Whenever the connection is being heavily utilised (for example during downloads or a speed test) pfSense reports up to 15% packet loss (and the gateways on the dashboard go into yellow "warning" mode). When performing less strenuous tasks (such as streaming video), the packet loss value is still showing around 5%.

Download speeds don't seem to be affected as we consistently get close to the line sync speed.

Is this anything to be concerned about? Surely there should be no (or very little) loss at all, even under heavy load.

Try setting the monitoring IP to something other than the gateway IP, so 8.8.8.8 for example is commonly used.
That gives you a better idea of actual connection quality. The ISP gateway usually doesn't guaranty ping response.
