I am attempting to set up an LTE modem which is stuck behind NAT. I understand that this is a typical problem but I couldn't find a solution in the setting of multiwan.

    Setup:

WAN-DHCP: IPv4 dynamically assigned, reachable from internet
WAN-LTE: private address behind CGNAT (not able to be reached from the internet)

I think my solution would involve setting up a VPS in a data server which would provide an IP address reachable from the internet.

My question is how do I set up my network to that the VPS IP address only attaches via the LTE gateway. This is only a failover (backup) method to access the network.

Is there a better way, perhaps ssh forwarding regardless of gateway?

    Thanks for helping design this.

Your network has to establish the connection, since it has a dynamic IP, for instance a VPN to the VPS. For stating the gateway to be used for that connection you have to add a static route for the VPS IP and select the LTE gateway.
