      Our Netgate SG1100 suddenly stopped working. Upon physical inspection, we noticed the WAN, LAN, and OPT ports are not lighting up when plugging in cables. When viewing the console info we see the OS boots up just fine and our configuration is intact. Ethernet chip/hardware issue?

        It does sounds like it might be.

        Try running at the command line: etherswitchcfg
        If it complains there is no switch device that's a problem.

        Make sure you do a full power cycle on it to reset the switch chip if you have not already. Rebooting does not do that.
