Does anyone know the proper way to setup nextdns CLI with pfsense ? I tried to do it as stated but nextdns cli tells me port 53 is already in use ?

@Buford-0 While these intructions are for OPNsense, it should provide enough guidance... How-to: NextDNS + OPNsense Firewall

Netgate 4100 and HPE InstantOn network at home

I did find this but not sure if it works.
In one of the post "you need to allow DNS Traffic to Firewall from the other Interfaces"

I am looking to set this up myself.

There is a simpler way, using an alternative DNS daemon. It's fully compatible with NextDNS (or any DNS upstream really).
