I am unable to install pkgs or check for updates. This is a fairly fresh install of pfsense

Check if pfSense can resolve host names with Diagnostics > DNS Lookup.

If not verify its DNS settings in System > General.

It is a DNS issue but why? I am using the same DNS server to access this page. But it fails when pfsense tries to use it?

I am running adguard DNS on 192.168.3.20

Note that the DNS server setting here is for pfSense only.
If you distribute pfSense as DNS for your devices via DHCP, you have to enable the forwarding mode in the DHCP Resolver settings.

Do I need to do anything under DNS forwarder?

You disabled the Resolver?? So that's the fault.
You don't need to run it though, but if not you have to change the "DNS resolution behavior" in the General Settings to use remote servers (the DNS server, you've stated).
