I'm just wondering…I'm running 1.2.3 RELEASE on an ESXi 4.1 host and the performance generally hangs around 15% or so, but there's a spike to 50% every 10 minutes...exactly.  I'm wondering what this might be.

Log on to the console or an SSH terminal and run top, you should see what's consuming the CPU.

I tried this and didn't really see anything.  There was a short "RUN" listed for a second, but then it was gone.  There are also a few items listed for VMware-tools, which i thought was odd (to be a the top of top).  I'll keep searching.  I just thought that someone would know this.

Is it VMWare that's showing the spike as being at 50%, or the tools inside pfSense?
