        1.  I don't know.
        2.  No authentication is working in the package right now.  I know several people (myself included) that would be very happy if you posted a bounty to get this feature up and running again.
        3.  We do this on our production box using the whitelist/blacklist and IP addresses.  You can exclude certain IPs or subnets from the filtering.  Install the package and everything is pretty self explanatory.

          Why squid authentication its not working?

          Is the problem in pfSense's package interface or is in original package and free bsd?

          If I config directly in squid.conf it will work?

            Yes, but it wont stick. To make it stick edit /usr/local/pkg/squid.inc, then regenerate squid.conf by changing an option on the proxy config screen of the gui.

            Remember to back it up though, in case a jr. tech (or a forgetful you) upgrades the package.
