Weird audio problem with SIP phone and asterisk

Hello group!

In a attachment I have a picture of my network with PBX and pfsense boxes scheme.

Bellow are descriptions of a problem. Can someone explain what's going on. Since I cannot run packet capture forever on box with IP2.

1. ext 208 calls 201 audio is passing in both directions
2. ext 208 is calling GSM phone audio is passing in both directions
3. GSM phone is calling public telco number that rings on 201 audio is passing in both directions
4. GSM phone is ringing to public telco number that rings on 208 audio is going only from 208 to GSM not in opposite direction
5. GSM phone is ringing to public telco number that rings on 208 audio is going both direction but only if I run packet capture on IPSEC interface of Pfsense box with IP2
