Huge issue with squid reverse proxy

Hello
I use Squid reverse proxy for 10 servers.
Everything is working fine (some I'm able to sets Squid RP)
But for one server, I'm landing on other one the webserver is the good one (http port 80 on 192.168.x.y name toto)
The mapping is also right servicename.mydomain.com with toto webserver
I'm landing on 192.168.x.z !
I really don't understand.
