That definitely looks like a bad disk. (Or the disk doesn't support DMA but is being told to use it – or vice versa, it requires DMA but it's not present)

im just curious about the words zyxel0 kernel.  because i have two isp . one of the modem is zyxel. ant the other one is prolink

what do you think is the connection of the log to my zyxel modem?

            thx. really appreciate it.

Where it shows zyxel0, that's the hostname of the system doing the logging, that must be what you have under System>General Setup.

Yes as others noted, that's a dead drive, many times people find after an upgrade that their disk is flaky (during normal operation on most systems it doesn't get hit much).

Though you don't really need to be concerned with that, replace the drive and I'm sure that'll go away.

because i ddidnt have ide hard disk to test it out. i decided to install the pfsense 2.1.2 which is my previous version.

                the error now is gone.

It won't be gone for long. Reinstalling won't stop that kind of issue. It may prolong its life for some period (because just a power cycle could kick it back into shape temporarily), but I'd have a spare drive on hand as it's likely it won't last long.
