This is a system I have been maintaining and upgrading since pfSense was a beta 1.x.  I have tried removing Squid, deleting /usr/local/etc/squid, and re-installing.

      Here's my culprit:
FATAL: url_rewrite_program /usr/local/bin/squirm: (2) No such file or directory

Mar 3 03:01:01 kernel: pid 64743 (squid), uid 0: exited on signal 6 (core dumped)
      Mar 3 03:01:01 squid[64743]: url_rewrite_program /usr/local/bin/squirm: (2) No such file or directory
      Mar 3 02:54:49 dnsmasq[729]: reading /var/dhcpd/var/db/dhcpd.leases
Mar 3 02:53:13 kernel: pid 63805 (squid), uid 0: exited on signal 6 (core dumped)
      Mar 3 02:53:13 squid[63805]: url_rewrite_program /usr/local/bin/squirm: (2) No such file or directory
Mar 3 02:52:53 php: : SQUID is installed but not started. Not installing redirect rules.

      Mar 3 02:52:47 check_reload_status: reloading filter
Mar 3 02:52:46 kernel: pid 63383 (squid), uid 0: exited on signal 6 (core dumped)
      Mar 3 02:52:45 squid[63383]: url_rewrite_program /usr/local/bin/squirm: (2) No such file or directory
      Mar 3 02:52:45 php: /pkg_edit.php: Starting Squid

Any help in repairing this is appreciated.

Looks like you've got other packages installed - did you by chance install any FreeBSD packages yourself (like squirm)/

Maybe you should reinstall the system with a 1.2 and restore the backuped config. I did so with a 1.0 beta1 last week and the config was upgraded just fine (didn't have a problem with it though, but as it was an embedded install so switching the cf-cards was pretty easy to do).
