I currently have PFsense installed on an old HDD, and I will be surprised if it is still functioning in a year. Is there a recommended way to back up my configuration and keep a cold spare HDD?

Just keep a regular backup, so you can rebuild and restore quickly.

I'd recommend installing the nanobsd version if you're worried about a hd failing. I've never had a USB fail that remained stationary.

Doesn't the NanoBSD version not support all of the same packages? I am not so much worried about the cost of the drives, as I can fetch an 80GB spinner for $20 or so. I just don't want to get comfortable with the drive that is in there, because it probably doesn't have a lot of life left.

All you need to backup is the config.xml (Diag > Backup/Restore). You can reinstall + restore faster than you could image the disk back.
